Credit Card Authorization Form.
In the event that you wish to pay by credit card while you wait to have your credit application reviewed Ensure offers you the convenience of paying by credit card. Simply fill out the Credit Card Authorization Form and fax it to us at 515.252.2519. In the event that you wish to limit the amount that Ensure can charge to your card for any single purchase please fill in the amount on the form.

Customers wishing to arrange for service must complete a Work Order Agreement unless they have a current Support Agreement.
