The following application process involves completing two online forms, printing those forms and faxing them to Ensure Technologies’ New Accounts Department. The process typically takes about 20 minutes to complete. Click the links below to fill out electronic versions of the required forms (Reseller Credit Application and Electronic Commerce User Agreement). When finished, click the “Done and Print” button at the bottom of the document. Incomplete fields will be flagged to ensure you complete the form in its entirety.

There is a $100 non-refundable reseller application processing fee. This fee can be paid by credit card or check to:

A rebate of $100 will be applied after 120 calendar days after the account has been opened to the original credit card or the Ensure Technologies account (if no credit card was used) if net sales are $1,500 or more during the first 90 calendar days the account is open. The account must be current and in good credit standing to receive this rebate.

We will waive the $100 application processing fee if the responsible principal(s) of your company submit a signed personal guaranty with your reseller application.

Once you have completed and printed all the forms, simply sign the hard copies and fax to our New Accounts department at 515-252-2500. IMPORTANT! Please make sure you have all the necessary forms, that they are filled out completely and signed at the bottom before faxing to us.

After your application is processed, you will receive an e-mail containing your account information; sales team name and phone number; and an expanded list of Ensure Technologies’ service offerings, which means you are ready to conveniently place orders and view our electronic catalog at www.ensuretech.net.
